In this case my first priority is the safety and welfare of my client and his family. His wife has stated that she is afraid that he may cause harm to her and her 3 year old daughter. She also stated that he has previously tried to overdose on his medication, Ritalin. He is beginning to show signs of decompensation (the deterioration of mental health in a patient with previously well managed psychological problems, leading to diminished ability to think and carry on daily activities). He has stopped coming to the center, not eating, rarely gets out of the house, and sleeps all day. He is talking gibberish, and mumbles profanities. So I think our first course of action would be to have him placed in an inpatient program, so we can find out what is going on with him. This will also keep him under observation, so he doesn't harm himself and we know he can't harm his family either. In reviewing the side effects of the medication he is currently taking, some of the side effects are anxiety, loss of appetite, and trouble sleeping, feelings of paranoia, hallucinations, and Depression. Also overdose side effects can be the same, too, but can also be similar to psychosis or schizophrenia. So we may need to look into taking him off this medication or changing his medication. He may also need to be placed on a depression medicine, since he is also showing signs of depression.

The type of treatment I am recommending starting out is an inpatient treatment. This will give us time to have him tested to see if he has been using any illegal drugs. This will give us a chance to see if he has been using any other drugs besides his prescribed medications. It will also allow us to have him completely off his medications while so we can see if his medications are causing him to react in this way or if he has been trying to overdose again. If so we need to find out why he would want to do this.

The types of therapy that I think would help this client are talk therapy and Cognitive Behavioral Therapy. Talk therapy can help him deal with emotional baggage and resentment at the nagging and criticism he receives from those close to him. CBT focuses on the practical issues that often come with ADHD, such as work problems, poor time management, and disorganization.
