Question description

based on the following info fill out request for change.

OCIO provides data and voice network services to Ministry and broader public sector clients. OCIO has two Internet Gateways to provide Internet service to its clients. Primary one is located in Vancouver and backup is located in Victoria. All the Internet traffic goes through primary location and can be failed over (automatic and forced) to the backup location in case of faults with primary service. On Feb 9th, 2018 (during working hours) the primary Internet services failed in Vancouver. The failover of the Internet services didn’t happen automatically as designed leading to a major outage in the network. Further root cause investigation revealed a problem with a specific module on the core router (VANRC987). Network Operations team temporarily disabled the defective module in the router as a workaround. The defective module needs to be replaced to mitigate the problem. The vendor can deliver the new module in 4 days, it will take around 1 hour of work with an anticipated outage of around 15-20 minutes, as the core router will have to be restarted. John Smith has requested you complete a request for change (RFC) to install the new module. In the event there’s any problem encountered with replacing the module, the card will be removed and John will work with vendor to identify the reason and if it cannot be solved within the scheduled RFC time the RFC will have to be rescheduled. The back-out activity should not take more than 15 minutes. Management wants to do this change as per our regular change management guidelines for adding new hardware. Not completing the change will mean lack of in-hardware redundancy. Based on the above information please fill in the attached request for change (RFC) form with all the details and suggest a most suitable date and time for this RFC to be carried out.
