***NOTE: I am required to record a video for this assignment, which I will do on my own- however, I am unable during this time to complete this assessment as a whole due to working in times of crisis- I need all the help I can get!***The scenario I want to use from my personal experience is as follows and you can use your own words: In the nursing home, I used interproffesional collaboration with doctors, social workers, and other various members of the interdynamic team to determine if a patient was fit for hospice. As a nurse, assessment is always crucial in determining a patients needs especially when transitioning from palliative care to end of life care. As per my suggestion to the physician, he determined that hospice care would be the best option to keep this patient comfortable (The patient was 88 years old male with a history of end stage renal failure and lung cancer along with a past history of diabeters) due to the family's unwillingness to learn about hospice because of the denial of their family members decline, it was the job of the doctor to first speak to the patient about the transition and my job as a nurse to educate the family, provide compassion, information using brochures and pamplets so that they could go home and discuss these matters in private. Working with the pharmacist on getting the proper medications (morphine sublingual) and discontinuing other medications was also crucial toward the interproffesional dynamic because we work on an EMR system that correlates which medication is being delivered to which patient.
