ANSWER ONE: The main reason the U.S has more specialists than generalists physicians is due to income difference. The general physicians have less income than physicians who specialize in a certain field. Secondly, specialist offer specific and more specialized services hence bill more for the services. According to Sachan, (2013), the average basic-care doctors earn approximately $140,000 annually and incurs about $200,000 in debt. Although the speciality doctors incur more in debts, they make more than $250,000 annually. The variation in income level is very large while the debts are almost the same. These financial incentives attract more students to speciality physician than a general physician. Secondly, according to Leigh et al. (2014), physicians whose income is between $250,000 and $2999,999 annually were 98 percent more likely to be very satisfied as compared to those earning salary between $100,000 and $149,000. He concluded that being financial award contributed a lot to job satisfaction. This satisfaction further proves why most people choose to be specialist physicians. Unlike general physicians, a specialist concentrates more on the root of the problem and is more specific which in turn encourages specialization. Also, the services given by the two groups differ. The specialist deal with services such as surgery which give them better results regardless of the complexity of the issue being handled. Hopkins was one of the first universities in the US and placed great emphasis on departmental specialization and research productivity. The most powerful departments in any medical school became the ones that attract the most research dollars and produce the most clinical revenue by doing highly reimbursed medical and surgical procedures. Primary care teaching and practice has always been deeply devalued by medical centers because it does neither” ( Frances, 2017). This is true many hospitals and medical schools value students going into primary care are considered less prestigious. Another reason is due to the drive toward even more specialization in business continues, with the majority of business leaders saying their department or organization would be more effective if more of their subordinates were more specialist than generalist (Martin, 2007). One of the biggest reasons that there are few students going to specialties is for the simple fact that specialties make more than generalist physicians. Dr. Michelle Finkel, a former Harvard admissions officer who now is an independent consultant to students on medical school admissions says that the top three reasons that my clients choose specialization over primary care are financial, lifestyle and administrative (Kavilanz,2009).
