Purpose: The purpose of this paper is to define a clinical problem, review the literature for
appropriate research supported nursing interventions, critique current practice and provide a
plan for changing or adapting clinical practice.
Role: You are an RN working in a clinical environment. Each unit has its own ‘way of doing
things’. As an emerging leader, you are charged with questioning WHY things are done the
way they are. Is there literature to support the way things are being done or should changes
be made based on evidence?
Audience: When writing this paper, your audience is your peers. Write in a way that utilizes
professional terms, but also explains terms and concepts. Just as you are learning as you
compose the paper, you will want to write in such a way that a peer could understand what you
are presenting.
Guidelines:
1. Introduction. Introduce the reader to the concepts being presented and the issue to be
discussed. Describe the specific settings and the practices within the setting. (For
example, ‘a 40 bed med-surg unit in an urban area’).
2. Present the Clinical Problem. Identify a clinical problem in your practice [i.e., pain,
nausea, depression, anxiety, medication errors, delirium, sleep deprivation etc.] Note:
This is not a professional health care issue such as staffing, entry level of education,
etc.
o Define the problem as it is found in your practice environment (do NOT use
textbooks)
o Describe why this is a clinical problem in your environment
o Explain why you chose this problem to investigate
o Be very specific with examples.
3. Current Clinical Practice. Describe the current clinical practice relating to this
problem.
o Summarize the formal mechanisms for dealing with this problem or the formal
mechanisms that create the problem [i.e., policy manual, standard of practice,
clinical path, etc.]
o Summarize the informal mechanisms for dealing with the problem or that create
the problem [What the actual current practice is].
o Analyze the rationale for any discrepancies between the formal and informal
methods
4. Nursing Interventions. Identify three [3] nursing interventions from the literature, which
address this clinical problem. The interventions should be based on current literature
(literature should be no more than 5years). At least one intervention must be based on
research. These are therapeutic interventions, which means that they are focused on
improving patient care. Textbooks may be used as a citation source, but are not the
best sources.
o At minimum 6 sources of current literature are needed to support the
interventions- At least two [2] sources of literature must be primary research or
evidenced based practice studies.
o In your paper analyze the relationship between the research, other literature
and the clinical problem.
o Develop a hypothesis for a nursing intervention you would like to test to handle
the problem you chose.
5. Summary. Should conclude your assignment with a summation of the problem and
your interventions.
6. APA format. It is expected that the principles of good writing will be used. Including
grammar, punctuation, spelling and sentence structure. This paper must follow the
guidelines of APA format (6th edition).
The Therapeutic Nursing Interventions (TNI) paper is a significant paper in this course. Below

Review writing guidelines and watch the narrated powerpoint before beginning this
paper. This will help you understand the components of this assignment.
Select a problem that is a patient centered problem within your environment. If you are
in administration “talk” with your mentor about your ideas. However, your issue should
address a patient centered problem.
Use headings and sub-headings for sections to correspond to the grading sections. This
helps keep you focused and helps faculty evaluate the assignment.
This paper requires specific examples. Included in the introduction is a requirement for
“setting” and “practices within the setting”. Give specific information about the unit or
environment the problem occurs within. Give specific examples of the practices within
the setting related to the problem.
The “Current Practice” section asks for formal and informal mechanisms. This asks for
you to identify what is current policy [i.e.] What is in the procedure manual related to the
problem.] also called a formal mechanism and what is informally done [i.e. What the
nurses actually in practice do.] Sometimes the procedure manual for how something is
done differs greatly from what is actually done by the nurse on a day-by-day practice.
You are asked to provide a rationale for this difference or state no difference occurs.
You are charged with identifying 3 nursing interventions to address the problem you
identified. One of the interventions must be based upon research. Standard research
or data behind best practices. The other two interventions can be from research, current
literature or expert opinion. You are also asked to discuss any differences or identify
similarities in interventional support of the problem.
You are required to have 6 sources of professional literature to support your
interventions. 2 of the 6 must be research.
Don’t forget the very small section about “Identification of a Potential Research Study
related to the problem you identified or to the interventions. This is your idea, not what is
currently in the literature.
The paper should be able to be completed in a maximum of 7-8 pages not including title
page or reference page. APA format and good writing skills are a must! Be succinct and
specific. Avoid the use of colloquialisms or regional ‘sayings’, this is a professional
paper and should be written in such a manner.
