Client is a 35 year old female presenting with persistent anxiety and depressive symptoms Which became worst recently because of her marital problems and her being a caretaker for her mother. She reports inability to focus on her own life and take care of her children because of her anxious and depressed mood

The plan is client-centered therapy; explore client’s feelings with a goal to decreasing anxiety and ways to manage stress and self-care, process issues that are sources of anxiety and depression, decrease the fear of disappointment and failure, increase the sense of self-acceptance and insight.

  • Describe the client and identify any pertinent history or medical information, including prescribed medications.

I receive clinical supervision from my preceptor almost on a daily basis as he helps daily
to select the patients to attend to as I practice. The preceptor wants me to learn new things on a
daily basis and therefore supervision is on a daily basis. I do not have clinical supervision when
taking care of patients we have worked together before which is always one day a week. The
sessions are good as my preceptor is so supportive and loving. The preceptor gives me humble
time to practice under his clear directions. I am not under any pressure any single day I am being
supervised. The sessions make me learn a lot from my residential physician at the clinic. I always
miss the sessions because they are enjoyable. Preceptor brings humor to me and the patient hence
a free working environment.
The preceptor brings to my attention on the schedules he has planned and what is expected
of me. The preceptor also passes information from the facility management and my school too.
When I want to communicate to my school or the facility management I also use the preceptor and
the issues are addressed and attention is given to me. Also, my preceptor brings to my attention
the legal and/or ethical implications of my counseling sessions. Through my preceptor, I am now
aware of my legal and ethical responsibilities throughout my practice in the facility. From the
sessions, I have been able to handle different patients with special needs appropriately with or
without supervisions. I am able to observe the ethical codes of conduct during my work as a result
of having continuous preceptor sessions. I have also been able to plan my working schedules well
after I have known what is expected of me from each session I have with my preceptor.

Presenting the problem
The client I have selected is a female of 35 years of age who has presented herself with
persistent anxiety and depressive mood symptoms. Recently, the symptoms have become worse
because of her marital issues and her being the caretaker of her mother. She has the challenge in
focusing on her own life and take care of her children because of her depressive mood and anxiety.
This reveals to me that she feels that she is not reaching her potential. The client needs personalcentered interventions to help realize her potential and live a normal life accepting her current
situations and achieving what she wants to accomplish. The intervention method was
recommended after interviewing the client and I realized that her suffering is as a result of not
being supported by anyone in her life to achieve her dreams and obligations as a woman. There is
a lot of pressure from her living environment something she does not know how to handle it.
