POST ONE/please respond to this post.The client, Jose, is seeking treatment with three complaints: edema of the scrotum, urinary retention, and hematuria. Upon digital rectal examination, the doctor found his prostate to be enlarged. Two of his symptoms, urinary retention and hematuria, are related to benign prostatic hyperplasia (BPH). The doctor finding that his prostate is enlarged could also point to BPH. However, I’m not finding any information indicating that BPH would cause edema of the scrotum. Therefore, I believe Jose may have prostate cancer..Prostate cancer is the most common type of cancer diagnosed in male client’s, aside from skin cancers. In early stages, the client may not experience any signs or symptoms. Typically, the first symptoms experienced are related to urinary elimination, such as urinary retention, urgency, nocturia, difficulty urinating, and hematuria. Jose is experiencing two of these symptoms (urinary retention and hematuria). When evaluating a patient and obtaining a health history for prostate cancer, it is important to ask about changes in sexuality, which can include if the client is experiencing any scrotal pain or swelling (Jose is experiencing scrotal swelling). Jose did mention that he waited for a period of time while experiencing symptoms before seeing his doctor. This is important, because hematuria is more commonly associated as a late symptom of prostate cancer, at least if there are large amounts of blood in the urine (Ignatavicius et al., 2021). In relation to the doctor finding that his prostate is “enlarged,” this is more in line with BPH, as the prostate is typically a consistent shape, rubbery, and larger than normal. In prostate cancer, the prostate is more likely to be hard and firm, and possibly contain a lesion (Sawyer Sommers, 2018). Specifically because Jose complains of scrotal edema, I believe his symptoms are more likely to be prostate cancer.In providing multidimensional care for Jose, the first important thing is to get an interpreter, as Jose speaks limited English. This will ensure that he is able to understand what the nurse is saying, and the nurse is able to understand what Jose is trying to say as well. I would also want to find out if there are any spiritual or cultural beliefs or practices that would affect the approach to care. As the nurse, I would also focus on Jose’s mental health to determine if his symptoms are causing any depression or anxiety, and how they interfere with his daily life. Guidance can be provided to Jose on how to manage these symptoms and interferences. If he does indeed have prostate cancer, this can be very difficult news for someone to learn and can also affect their family members (Jose has a wife and 5 young kids). I would assess what support Jose and his family have or are aware of regarding a cancer diagnosis (such as friends, spirituality or religious beliefs, support groups, or mental health professionals). I would prepare Jose through education on any diagnostic procedures his physician may perform, and education on what disease process he is diagnosed with (Ignatavicius et al., 2021).Sources:Ignatavicius, D. This is important, because hematuria is more commonly associated as a late symptom of prostate cancer, at least if there are large amounts of blood in the urine (Ignatavicius et al., 2021). In relation to the doctor finding that his prostate is “enlarged,” this is more in line with BPH, as the prostate is typically a consistent shape, rubbery, and larger than normal.
