A comprehensive history taking process allows a healthcare consumer to receive the best
healthcare since the clinician will know about previous conditions and any underlying issue that
is affecting the patient’s health. As Ponathil et al. (2018) illustrate, health practitioners are
required to keenly evaluate if the patient understands the medical questions they are asked and if
the patient provides reliable information. This is to allow proper collection of medical history
that will allow the clinician to give a correct diagnosis. During discussion of the current
condition, it becomes important to inquire on any previous conditions that may be chronic,
history of childhood diseases, previous hospitalization, and any surgical history. However, it is
not always that proper medical history is taken. There are various challenges or barriers that
bring about limitations in taking a medical history of patients. These barriers include
overwhelmed staff, lack of time, emergency cases, language barriers, a patient having medical
histories they are embarrassed about and patient not being used to a new session with a different
clinician. Therefore, this paper will seek to address these issues and how they are barriers for me
when collecting health history which is a vital aspect healthcare provision.
