ANTIPLATELET DRUGS;Approach to the management of thrombosis; Both Antiplatelet drugs and anticoagulants are effective in prevention of arterial, cardiac and venous thromboembolism. Antiplatelet drugs are preferred for routine cardiovascular prevention becausethey are less likely to cause excessive bleeding. Anticoagulants are however more effective in treatment of venousthromboembolism and peripheral arterial embolism and are indicated in specificcircumstances such as atrial fibrillation. Thrombolytic agents have a main role in treatment of acute thrombotic arterialocclusion especially coronary thrombosis.ASPIRINPharmacology; All the pharmacological effects of aspirin are based upon inhibition ofprostaglandin synthesis. Aspirin is used as an antiplatelet drug in doses lower than those employed foranalgesia. Aspirin acetylates and inactivates cyclo-oxygenase, a key enzyme in the plateletbiosynthetic pathway for the pro-aggregatory prostaglandin thromboxane A2. The interaction of aspirin with cyclo-oxygenase is irreversible. As platelets areanucleate, the recovery of thromboxane synthesis is dependent uponappearance of new platelet population in the circulation. Recovery from the effect of aspirin takes about 10 days. Aspirin also inhibits endothelial cyclo-oxygenase leading to reduction ofprostacyclin synthesis; an effect that is theoretically undesirable.Effect on haemostasis; Administration of aspirin results in
