Case 1 The patient should be immediately taken for insertion of Percutaneous Trans-luminal Coronary Angioplasty (PTCA) within 4 to 6 hours from the onset of angina to enable the much needed urgent perfusion of the myocardium (Jayasinghe, 2008, p. 32). Immediate PTCA minimizes further ischemia in the heart muscles and to maximize the chances of PTCS succeeding (Jayasinghe, 2008, p. 33). The insertion of a PTCA is accomplished by the insertion of a guide-wire into the occluded coronary artery with an inflated tiny balloon fixed over the edge of the guide-wire with the aim of opening up the blocked arteries and restoring adequate perfusion of the heart tissue (Jayasinghe, 2008, p. 33).
