Tetralogy of Fallot (TOF) involves four heart malformations that present together: A ventricular septal defect (VSD) is a hole between the ventricles of the heart. “The defect is centered around the 'outlet septum', the most superior aspect of the septum, and in the majority of cases is single and large. In some cases septal hypertrophy can narrow the margins of the defect,” (Wikipedia, 2007). Pulmonic stenosis, a ventricular outflow obstruction, caused by a narrowing at or just below the pulmonary valve. The stenosis is the major cause of the other malformations that must compensate for the pulmonic stenosis. The degree of stenosis varies in patients with TOF, and determines the severity of symptoms (Wikipedia, 2007).

The degree of override varies between 5-95% of the valve being connected to the right ventricle (Wikipedia, 2007).Right ventricular hypertrophy defined as right ventricular enlargement, causing a characteristic boot-shaped appearance on a chest X-ray. This is referred to as a secondary abnormality, because the level of hypertrophy usually increases with age (Wikipedia, 2007).Due to the mixing of oxygenated and deoxygenated blood between the septum and the overriding aorta, TOF results in low overall oxygenation.
