Patient TC is a 53 years old, female, Filipino, Born again, housewife and a resident of Pandacaqui resettlement. She was admitted for the 2nd time at AUFMC. Date of Admission: November 29, 2012 Chief complaint: Difficulty urinating History of Present Illness: 3 days prior to admission the patient experienced vomiting accompanied by fever and chills. The vomiting had occurred during nighttime for 3 days. It is described to be continues and the vomitus is watery and moderate in amount. She took 1 tablet of omeprazole but provided no relief. Fever occurred intermittently with the highest temperature reading of 38’c. It was managed by intake of 1 tablet paracetamol and provided temporary relief.
